The short answer: a normal TSH means your thyroid replacement dose is probably right. It does not mean your symptoms are imaginary, and it does not mean the thyroid is the only thing that could produce them. Roughly 5% to 10% of people treated for hypothyroidism report ongoing symptoms with a normal TSH. The usual explanations fall into four buckets: the dose is technically adequate but the drug is not being absorbed consistently, the number is in range but not where you personally feel best, something else entirely is causing the symptoms, or the symptoms are real and the thyroid has genuinely been optimized.

Working through them in order is more productive than arguing about the number.

What does a normal TSH actually tell you?

TSH is thyroid-stimulating hormone, made by the pituitary. It is not a thyroid hormone. It is the pituitary's instruction to the thyroid, and it moves in the opposite direction from thyroid hormone levels: when thyroid hormone is low, TSH rises to push harder.

The American Thyroid Association describes the commonly used range as about 0.4 to 4.0 mIU/L, and current guidance for people on levothyroxine is to adjust the dose so TSH sits within that range while symptoms resolve.

Two caveats matter. TSH is a pituitary signal, which makes it an indirect measure of what your tissues are receiving. And a "normal range" is a population statistic, built from where 95% of a reference population falls. It describes a crowd, not you.

Hypothyroidism is common enough that this affects a lot of people. The most-cited U.S. prevalence estimate, from NHANES, puts total hypothyroidism at roughly 4.6% of the population, most of it subclinical (Hollowell et al., JCEM 2002).

Is your medication actually being absorbed?

Start here, because it is the most common fixable problem and it is almost always missed at home rather than in the lab.

Levothyroxine absorption is fragile. It needs an empty stomach and acidic conditions, and a long list of ordinary things interfere. The NIH Office of Dietary Supplements notes that product labeling advises against taking levothyroxine within four hours of iron supplements, because iron reduces its absorption.

The practical rules:

  • Take it on an empty stomach, typically 30 to 60 minutes before food, or at bedtime at least four hours after eating. Pick one and keep it constant, because consistency matters as much as timing.
  • Separate by about four hours: calcium, iron, multivitamins containing either, and antacids.
  • Watch coffee. Taking levothyroxine with coffee reduces absorption. Wait.
  • Watch acid reducers. Proton pump inhibitors raise stomach pH and can reduce absorption, and they are frequently taken long-term without review.
  • Keep the brand or generic consistent. Switching manufacturers can shift your level enough to matter. Our comparison of levothyroxine versus Synthroid covers why.

If your routine has been inconsistent, fix it, hold it steady for six to eight weeks, and retest before concluding anything else.

Is "normal" the same as optimal for you?

Not necessarily, and this is where a reasonable conversation with your clinician can happen.

A TSH of 3.8 and a TSH of 0.9 are both inside the reference range and are not the same physiological state. Some people on replacement report feeling better in the lower half of the range. Many guidelines allow targeting the lower portion of the range for symptomatic patients, as long as the level is not pushed into the suppressed territory that carries real risks to bone density and heart rhythm.

What is worth asking for: a full panel rather than TSH alone. TSH plus free T4, and in some cases total or free T3, gives a fuller picture than a single number, particularly if TSH and symptoms keep disagreeing. Our guide to thyroid blood tests covers what each one measures.

What is not worth doing: chasing a specific number you read online. There is no evidence-supported universal "optimal TSH" that applies to everyone.

What about T3 combination therapy?

This is the question that dominates patient forums, and it deserves an honest answer rather than a partisan one.

The theory is straightforward. Levothyroxine is T4, a prohormone your body converts to the active hormone T3. If someone converts poorly, they could have a normal TSH and normal T4 while running low on the hormone that actually does the work. Adding liothyronine (T3) would fix that.

The evidence is less straightforward. More than twenty years of randomized controlled trials have largely failed to show that levothyroxine plus liothyronine outperforms levothyroxine alone on average across treated patients. At the same time, guideline bodies have acknowledged that a subset of patients report persistent symptoms on monotherapy, and some now permit a carefully supervised trial of combination therapy in patients who remain symptomatic after other causes have been excluded (British Thyroid Association / Society for Endocrinology joint statement, 2023).

Two practical points. T3 has a short half-life and produces peaks and troughs that T4 does not, so it requires closer monitoring. And a trial is a trial: it should have a defined duration, a predefined way to judge whether it helped, and a plan to stop if it did not.

What else produces these exact symptoms?

This is the bucket that gets skipped most often, because once someone has a thyroid diagnosis every subsequent symptom gets attributed to it. Fatigue, brain fog, weight change, low mood, hair thinning, and cold intolerance are among the least specific symptoms in medicine.

Worth ruling out:

  • Iron deficiency. Ferritin can be low with a normal hemoglobin and a normal complete blood count. Fatigue and hair shedding are classic. Ask for ferritin specifically.
  • Vitamin B12 deficiency. Common with autoimmune thyroid disease, metformin use, and vegetarian diets. Produces fatigue, cognitive fog, and neuropathy.
  • Obstructive sleep apnea. Weight gain from untreated hypothyroidism can trigger or worsen it, and then it produces the same exhaustion and fog after the thyroid is corrected.
  • Depression. Overlaps with hypothyroidism symptom for symptom, and treating one does not treat the other. Covered in detail at depression.md.
  • Perimenopause. Overlapping age range, overlapping symptoms, entirely different treatment.
  • Celiac disease. Associated with autoimmune thyroid disease, and it also impairs levothyroxine absorption, so it can cause the problem twice.

A practical sequence

  1. Fix the routine. Same time daily, empty stomach, four hours from calcium, iron, and antacids. Hold it for six to eight weeks.
  2. Retest properly. TSH and free T4, drawn before your morning dose, at a consistent time of day. TSH has a daily rhythm and is higher in the early morning.
  3. Look elsewhere. Ferritin, B12, complete blood count, and a sleep apnea screen if you snore or wake unrefreshed.
  4. Discuss the target. If everything else is clean and TSH sits in the upper range, ask whether a modest adjustment toward the lower half is reasonable for you.
  5. Consider referral. If symptoms persist after all of that, an endocrinologist is the right place to have the combination therapy conversation.

The bottom line

A normal TSH means the pituitary is satisfied. It does not mean every question has been answered. Between 5% and 10% of treated patients feel unwell with a normal number, and the productive response is a systematic one: verify absorption, retest correctly, look hard at iron, B12, sleep, and mood, and only then debate the target and the possibility of T3.

What you should not accept is being told the number is fine and the conversation is over. And what you should not do is start adjusting your own dose or buying thyroid hormone without supervision, because over-replacement carries documented risks to bone density and heart rhythm that under-replacement does not.
