The short answer: The FDA levothyroxine label names the interfering agents directly. Calcium carbonate, ferrous sulfate, sevelamer, and lanthanum must be taken at least 4 hours apart from the dose, and bile acid binders such as cholestyramine and colesevelam carry the same 4 hour rule. Coffee, soybean flour, cottonseed meal, walnuts, dietary fiber, and grapefruit juice all reduce or delay absorption. Biotin is a separate case: it does not change your thyroid, but it distorts the blood test that measures it.

What does the FDA levothyroxine label actually list?

The prescribing information names agents and intervals directly. The Synthroid label on DailyMed gives one overall instruction in its Dosage and Administration section: "Administer SYNTHROID at least 4 hours before or after drugs known to interfere with SYNTHROID absorption."

Section 7 then breaks the list into groups by mechanism. Some agents bind the hormone into a complex it cannot absorb. Which supplements need a 4 hour gap?

Calcium and iron are the two that come up most often, and both appear by name on the label. Calcium carbonate can form an insoluble complex with levothyroxine. Ferrous sulfate forms a poorly soluble iron and thyroxine complex. For both agents the label instruction is the same: "Administer SYNTHROID at least 4 hours apart from these agents."

MedlinePlus repeats the rule for patients: take calcium or iron "at least 4 hours before or 4 hours after you take levothyroxine."

The size of the effect is real. A 2021 systematic review in Pharmaceuticals found calcium reduced levothyroxine absorption by roughly 20 to 25 percent. A 2023 systematic review in Therapeutics and Clinical Risk Management reported a 31 percent drop with co-administered calcium carbonate, and a rise in average TSH from 1.6 to 5.4 mU/L over 12 weeks with iron supplements.

Two practical points follow. Most multivitamins contain calcium, iron, or both, so the gap applies to them as well. Antacids that contain aluminum hydroxide, magnesium hydroxide, or simethicone are on the same label list. Which prescription medicines bind levothyroxine in the gut?

The bile acid sequestrants and ion exchange resins are the strongest binders on the label. That group includes cholestyramine, colestipol, colesevelam, and sodium polystyrene sulfonate. Here the label wording shifts slightly: "Administer SYNTHROID at least 4 hours prior to these drugs or monitor TSH levels."

The binding is substantial. The 2023 review found colesevelam cut the thyroxine AUC to 3.8 percent of the value seen with levothyroxine alone. Phosphate binders behave the same way. Sevelamer reduced thyroxine AUC to 50.4 percent and lanthanum to 59.1 percent. Both sit in the same 4 hour group as calcium and iron on the label.

Two more prescription agents deserve a mention. Sucralfate binds levothyroxine directly, with a 71.7 percent reduction in peak absorption reported in healthy volunteers. Orlistat gets its own instruction on the label: monitor patients treated with both orlistat and Synthroid for changes in thyroid function.

Do proton pump inhibitors and antacids matter?

Yes, though the mechanism differs. Levothyroxine tablets need an acidic stomach to dissolve. Proton pump inhibitors, sucralfate, and antacids all appear on the label as agents that may reduce absorption by raising gastric pH.

The 2023 review measured the effect: peak concentration fell to 87.32 percent and 12 hour AUC to 83.22 percent with a proton pump inhibitor on board. The label does not set a fixed separation interval for this group. Instead it directs clinicians to monitor thyroid function, because the problem is the stomach environment rather than a timed collision between two products.

Which foods and drinks interfere?

The label has a short drug and food interactions section, and it is worth quoting: "Soybean flour, cottonseed meal, walnuts, and dietary fiber may bind and decrease the absorption of SYNTHROID from the gastrointestinal tract. Grapefruit juice may delay the absorption of levothyroxine and reduce its bioavailability."

Coffee is not on the FDA list, but the evidence is well documented. The espresso work behind this began with case reports of high TSH in people who swallowed the tablet with coffee. The 2021 Pharmaceuticals review reported that coffee lowered average thyroxine levels by 29 to 36 percent and delayed absorption by 38 to 43 minutes. The 2023 review put the reduction near 32 percent.

For grapefruit juice, the measured effect is smaller: a 13 percent drop in the 4 hour thyroxine AUC. Fiber sits in a similar range, with psyllium reducing absorption by about 9 percent in one study.

The American Thyroid Association guidelines for the treatment of hypothyroidism handle food with timing rather than avoidance. Recommendation 3a asks that levothyroxine "be consistently taken either 60 minutes before breakfast or at bedtime (3 or more hours after the evening meal) for optimal, consistent absorption." Recommendation 3b adds that levothyroxine should be separated from interfering medications and supplements, and notes plainly that "a 4-hour separation is traditional, but untested." Does biotin change your thyroid or only your lab results?

Biotin belongs in its own category. It does not interfere with absorption and it does not change how your thyroid works. It interferes with the test.

The FDA warns that "biotin, often found in dietary supplements, can significantly interfere with certain lab tests and cause incorrect test results." Many hormone assays use a biotin and streptavidin binding system, so extra biotin in the sample throws the reading off.

The result pattern is distinctive. A 2023 case report in the Journal of Medical Case Reports describes results that "mimic hyperthyroidism with low TSH, high TT3, high fT3, and high fT4." It can even mimic Graves disease, because biotin also disturbs the antibody assays. The authors stress that the patient's thyroid is fine: the abnormal numbers are not reflected in the person. Doses matter here. Typical intake needs sit around 30 to 70 mcg a day, while hair, skin, and nail supplements often supply 10,000 mcg. The report suggests holding biotin for 48 to 72 hours before thyroid testing.

What about iodine and kelp supplements?

Iodine and kelp are also a different problem. They do not block your tablet. They act on the gland itself, and too much can push thyroid function in either direction.

The American Thyroid Association statement on excess iodine "advises against the ingestion of iodine and kelp supplements containing in excess of 500 mcg iodine daily for children and adults and during pregnancy and lactation." It adds that intake above 1,100 mcg per day, the tolerable upper limit, "is not recommended and may cause thyroid dysfunction."

The statement also names who is most at risk: "Infants, the elderly, pregnant and lactating women, and individuals with preexisting thyroid disease are susceptible to adverse effects of excess iodine intake and exposure." If you already take thyroid medication, you are in that last group.

The bottom line

The interference list is shorter and more specific than most people expect. Calcium, iron, sevelamer, lanthanum, and the bile acid binders get a 4 hour gap on the FDA label. Acid reducers and orlistat get monitoring instead of a timed gap. Soy flour, cottonseed meal, walnuts, fiber, grapefruit juice, and coffee reduce or delay absorption, and consistency matters more than avoidance. Biotin and kelp are separate issues: one skews the test, the other acts on the gland.
